Enrollment Forms

Enrolling in the Exceptional Family Member Program, requires completion of Form DD2792 (Exceptional Family Member Medical Summary). The form is completed by both the sponsor or parent and a qualified medical provider.

Any child identified or requiring services through an Individualized Family Support Plan (IFSP), or an Individualized Education Plan (IEP) are required to complete Form DD2792-1, in addition to the Medical Summary (Form DD2792).

A sponsor or family member may decline case management services provided by the local EFMP installation. If the sponsor or family member so chooses, he or she must sign an EFMP Declination of Services form. Once the Declination of Services form is processed, the local EFMP office will have no further contact with the individual or family. All future contacts will be conducted through HQMC.

UPDATES

Every 3 years, Exceptional Family Members are required to update their conditions by resubmitting a completed DD2792 to the local EFMP office. The update provides HQMC the opportunity to reevaluate the original condition. Children receiving services through an IFSP or IEP must complete Form DD2792-1as well.

Respite Forms

Respite care is designed to provide short periods of rest and relaxation to the caregivers of Exceptional Family Members (EFM’s) with significant needs. EFMP enrolled families, who are determined eligible to receive respite care by Headquarters Marine Corps EFMP, are able to receive reimbursement for up to 20 hours per month of respite care, not to exceed 6 hours per day, which is time designated for leisure. EFM’s eligible to utilize respite care are determined by Headquarters Marine Corps EFMP. Respite care cannot be used for preschool, seeking or working a job, medical appointments, transportation, house cleaning, therapeutic camps or attending college. 

Care is provided by the skilled care provider of your choice. Care may be provided in your home or community agency. Eligible families must provide proof of respite care provider's skill level to the EFMP office priot to utilizing care.

Payment amounts are determined by the installation EFMP  Manager based on the level of care needed for each child.


How To Get Started

The Exceptional Family Member Program is comprised of 3 components:

Eligible Exceptional Family Members Include:

  • Dependent(s) of an active duty service member
  • Spouse, child, or parent
  • Resides with the sponsor (51%)
  • Requires medical specialty care or
  • Chronic medical condition requiring more than annual visits with a primary care physician
  • Eligible for early intervention services (IFSP)
  • Eligible for special education or related services (IEP)

Required form needed for registration:

  • DD Form 2792 (all family members)
  • DD Form 2792-1 (children with an Individualized Family Services Plan or Individualized Education Program)

Supplemental documentation needed as applicable (i.e., IFSP, IEP, etc.)

Completed forms can be submitted to the local EFMP office or Headquarters EFMP at hqmc.efmp@usmc.mil.

Assignment Coordination

When a family member's specific medical or educational needs are documented via the DD2792 and DD2792-1, HQMC EFMP Assignment specialist use the information to match documented needs to the capability of the proposed PCS location. While the military's mission guides these assignments, when MMOA/MMEA/RAM and EFMP work together, the goal is to find an assignment that supports mission, career progression and family member needs.

Enrollment in EFMP ensures consideration of a family member's needs during military assignments with the goal to place families in locations that can support those needs.

EFMP offers various support services, including information, referrals, and case management.  Families can participate in EFMP events to build connection and support. Some families may be eligible for respite care.

EFMP helps facilitate necessary healthcare access during travel or relocation by making medical case management referrals to TRICARE.

Some individuals enrolled in EFMP will be eligible for priority housing. This status will be listed on your official enrollment letter and is part of the initial and updated enrollment process.

Families should update EFMP enrollment every 3 years or when there are changes in the family member's medical or educational needs.

If the family member is no longer a dependent, update DEERS and contact hqmc.efmp@usmc.mil to request disenrollment.  If the family member’s medical and/or education condition has resolved, update DD 2792 and DD 2792-1, if applicable and submit with a request for disenrollment.  If the condition still exists, we will not be able to facilitate the disenrollment, as EFMP is a DoD mandatory enrollment program, when qualifying conditions are present.

The Exceptional Family Member Program has Family Support professionals who can assist families in navigating the program, accessing resources, and learning how to advocate for their needs.

Personal and medical information within the EFMP is treated with strict confidentiality and follows privacy regulations.

Families can address disagreements or concerns about EFMP decisions through established channels, often involving the EFMP office or higher-level appeals processes. 

Respite Care is a non-entitlement benefit intended to provide brief rest periods for the primary caregiver(s). Families select hire and pay for their respite care provider and seek reimbursement from EFMP.
